What a UK tattoo consent form must cover
There is no single national form - your local council's byelaws set the detail - but the common core every assessor expects is:
- Age & ID verification - date of birth and the type of photo ID you checked. Tattooing under-18s is a criminal offence.
- Medical history - conditions that affect healing or risk (diabetes, blood-borne viruses, allergies, blood thinners, pregnancy, skin conditions).
- Procedure details - what was done and where on the body.
- Informed consent & declaration - confirmation the client is sober, fit to proceed and understands aftercare.
- Signature and date, plus the artist's name.
Why age verification is the part that matters most
The Tattooing of Minors Act 1969 makes it illegal to tattoo anyone under 18 (other than by a medical practitioner). "They told me they were 18" is not a defence if you didn't check ID.
